Mercado Livre has started a pilot project for selling medications in neighborhoods of São Paulo with delivery in up to three hours operated by pharmacy Cuidamos, offering over-the-counter medications such as analgesics, antipyretics, and vitamins with support from pharmacists during the purchase

Mercado Livre has just taken a step that could transform the way Brazilians buy medicine. The largest marketplace in Latin America has begun selling medications in São Paulo as a pilot project, with delivery in up to three hours after the order is placed online, marking the company’s official entry into the Brazilian pharmaceutical retail market.

According to Jovem Pan, the operation started on a small scale, serving neighborhoods like Vila Mariana, Paraíso, and Itaim, and operates through pharmacy Cuidamos, acquired by Mercado Livre last year. The available products are over-the-counter medications, including analgesics, antipyretics, antacids, digestive aids, and vitamins, and orders are monitored for provenance and supported by pharmacists who guide consumers during the purchase.

Online medication sales: how the Mercado Livre pilot project works

The pilot project for selling medications operates integrated with the Mercado Livre app and website, but with an extra layer of care that differentiates the purchase of medicines from buying any other product on the platform.

Orders undergo monitoring of the provenance of the medications and have pharmacists available to guide consumers before finalizing the purchase.

At this initial stage, the service is only available in a few neighborhoods of the São Paulo capital. The choice of Vila Mariana, Paraíso, and Itaim as initial regions indicates that Mercado Livre is testing the model in areas with high order density and already established logistical infrastructure.

Expansion to other neighborhoods and cities directly depends on the performance of the initiative, but the company has already indicated that the plan is to gradually expand as the pilot results come in.

Delivery in up to three hours changes the logic of buying medications

The most evident differentiator of the project is speed.

Delivery in up to three hours after the order places Mercado Livre in direct competition with neighborhood pharmacies and delivery apps, offering the convenience of not leaving home combined with a timeframe that covers most urgent everyday situations.

For those needing an analgesic, an antipyretic, or an antacid and who do not want to face lines or travel, delivery in up to three hours transforms Mercado Livre into a real alternative to physical pharmacies.

The promise is especially relevant for the elderly, people with reduced mobility, or those simply too ill to leave home, situations where going to the pharmacy is precisely the problem that online medication sales solve.

Pharmacy Cuidamos is the foundation that enables the operation

The entry of Mercado Livre into the pharmaceutical retail sector did not happen from scratch.

The company acquired pharmacy Cuidamos last year in a strategic move that secured the sanitary license, regulatory infrastructure, and operational knowledge necessary to legally sell medications in Brazil.

Pharmacy Cuidamos acts as the backbone of the entire operation. It is from there that the over-the-counter medications are dispatched, it ensures the provenance of the products, and its pharmacists provide technical support during the purchase.

Without the acquisition of pharmacy Cuidamos, Mercado Livre would have needed to build all this regulatory structure from scratch, which would delay market entry by years.

The move shortened the path and positioned the company to test the model while competitors are still planning their moves.

Over-the-counter medications are the starting point

The pilot project works exclusively with over-the-counter medications.

The list includes analgesics, antipyretics, antacids, digestive aids, and vitamins, categories that represent the majority of purchases made in pharmacies for simple and common everyday problems.

The decision to start with over-the-counter medications is strategic for two reasons. First, the regulation is simpler, as it does not require validation of a medical prescription for each sale. Second, these are the products with the highest volume and frequency of purchase in Brazilian pharmacies.

If Mercado Livre can prove that selling medications works well in this category, the path to include prescription medications in the future becomes shorter, although the company has not yet announced concrete plans in this regard.

This move could change the way Brazil buys medications

Mercado Livre already dominates categories such as electronics, fashion, and food in Brazilian e-commerce.

The entry into medication sales with delivery in up to three hours through pharmacy Cuidamos adds health to the portfolio and targets a market that is still predominantly physical in Brazil, where most people buy medications in person out of habit or due to a lack of reliable digital alternatives.

If the pilot in São Paulo works and the model is expanded, the impact on traditional pharmacies and pharmaceutical delivery apps could be significant, as Mercado Livre already has the user base, logistics, and brand trust that took years for other competitors to build.

The combination of over-the-counter medications, delivery in up to three hours, and pharmacist support could be what is needed to normalize online medication purchases in Brazil.
