CNH Exams Now Have a Limit of R$ 180, Reducing Disparity That Reached R$ 550 in Some States
The end of 2025 brings relevant changes for those who want to obtain their CNH and have been delaying their plans due to costs.
In addition to the removal of the requirement for driving schools, which can reduce issuance by up to 80%, the exams required for obtaining and renewing the license now have a national ceiling of R$ 180.
The measure is already in effect and applies to psychological evaluation and physical and mental fitness exams required by the Brazilian Traffic Code (CTB).

What Happened and Why It Caught Attention
The changes are outlined in Ordinance No. 927/2025, published by the National Traffic Secretary (Senatran) in an extra edition of the Official Gazette on the night of Friday (12).
The central point is the establishment of a national limit for the total cost of the required exams, which reduces the disparity of values between states.
In practice, the change directly impacts the wallet of those who will issue or renew their CNH, especially where prices were higher.
What Changes in Practice for Those Getting or Renewing the CNH
The ceiling of R$ 180 now applies to the set of procedures related to the medical exam and psychological evaluation necessary for licensing.
This is expected to lower the average price charged for these services in various regions, as there were locations with costs well above this level.
The rule also aims to standardize national costs, reducing differences that were determined by local practices in each state.
How Was Price Variation Before the National Ceiling
Prior to the change, the exam prices varied according to rules and accreditations related to each region’s Detran.
This difference created a significant disparity in the total cost of exams, ranging between R$ 200 and R$ 550.
In São Paulo, the cost reached nearly R$ 265, considering both psychological evaluation and medical exam.
In Minas Gerais, expenses were around R$ 443, while in Rio Grande do Sul, the cost was lower, at R$ 196.
In the Federal District, the total could reach around R$ 550, with approximately R$ 295 for the medical exam and R$ 255 for the psychological exam.
What Are the Rules, Deadlines, and Conditions
The ordinance establishes a limit of R$ 180 for certain values related to the medical exam and psychological evaluation, and not for each individual procedure.
This means that the ceiling applies to the total charged within the defined scope and not as a maximum price separated by phases.
When there is an equal distribution of the payment, the amount for each professional, doctor, and psychologist is limited to R$ 90.
How Payment for Exams Works
The fee for the exams is paid directly to the professional or the clinic accredited by Detran.
With the national ceiling, the remuneration becomes conditional on the total limit defined for the set of procedures.
This affects the current billing model practiced by clinics and professionals providing evaluation and assessment services.
Points of Attention and Common Questions
Health professionals have expressed dissatisfaction with the change, citing the term “imposition of values without technical study, dialogue, and cost analysis”.
The National Mobilization of Traffic Doctors and Psychologists, with members from the Brazilian Association of Traffic Medicine (Abramet) and the Brazilian Association of Traffic Psychology (Abrapsit), states that the measure may affect accessibility structures and standards, including NBR 9050.
The organization also reported that it intends to take legal action in the process, in different instances, including requests for precautionary measures.
The debate is likely to gain traction because it involves costs to citizens, professional remuneration, and the way assessments are conducted in the traffic system.
At the center of the change is the new ceiling of R$ 180 for exams related to the CNH, now in effect, with the promise of reducing price differences between states and making the process more accessible.
At the same time, the topic broadens the discussion on service feasibility and reactions from accredited doctors and psychologists, with the possibility of judicial disputes affecting the next steps.
