CNH enters the debate in Congress with a manifesto warning that diseases do not appear in fines; entities cite 38,253 deaths in traffic in 2024 and a direct cost of R$ 400 million to SUS
The CNH has become the target of criticism from more than 35 medical entities after Congress established a special committee to analyze Provisional Measure No. 1,327/2025, which provides for automatic renewal without a physical and mental fitness exam.
The entities claim that removing the requirement for the exam reduces the ability to prevent deaths in traffic, because health conditions that affect driving are not detected by inspections, radars, or fines.
What the MP changes in the renewal of the CNH
The Provisional Measure, published on December 10, 2025, changes rules of the Brazilian Traffic Code and affects the validity of the CNH and the way exams are conducted for obtaining and renewing the license.

Among the highlighted points, the MP allows the issuance of the CNH in digital format and establishes automatic renewal for drivers registered in the National Positive Driver Registry (RNPC), provided they have no recorded infractions, with exceptions provided.
Why the entities say the exam is decisive
The manifesto, led by the Brazilian Traffic Medicine Association (Abramet), argues that fitness to drive is not permanent and can change over time due to diseases, medication use, or clinical events that affect vision, reflexes, cognition, and motor skills.
Abramet highlighted that a driver can be unfit without any record of infractions, citing examples such as diabetes, heart conditions, epilepsy, sleep disorders, and neurological diseases, which do not appear in fines but impact safety on the road.
Who signs the manifesto and which areas are cited
In addition to Abramet, the document is signed by the Federal Council of Medicine (CFM), along with the regional councils, the Brazilian Medical Association (AMB), the National Federation of Physicians (Fenam), the Brazilian Medical Federation (FMB), and the Brazil Institute of Medicine (IBDM).
According to the entity, the group includes representatives from areas related to the care of traffic victims, such as emergency, intensive care, neurology, orthopedics, and rehabilitation, as well as specialties that deal with clinical conditions capable of interfering with the ability to drive.
Commission in Congress and data used in the debate
Congress established the special committee to analyze the MP, and the mentioned structure includes federal deputy Luciano Amaral as president, senator Dr. Hiran as vice-president, and senator Renan Filho as rapporteur.
The entities also cite numbers to support the warning: in 2024, Brazil recorded 38,253 deaths in traffic and nearly 285,000 hospitalizations, with a direct cost of approximately R$ 400 million for the Unified Health System (SUS).
They note that the data does not include long-term expenses, such as rehabilitation and social security benefits, which would increase the economic impact.
Exams, RNPC, and the exceptions provided in the MP
Under the MP, any doctor or psychologist will be able to conduct physical and mental fitness exams and psychological assessments, without the need for a link with Driver Training Centers (CFCs).
The maximum values for these exams will be set, and the document may be issued in physical or digital format.
According to the measure, drivers without infractions in the RNPC will have automatic renewal of the CNH, exempting them from new exams, except in cases of people aged 70 or older, drivers aged 50 or older with the right to only one automatic renewal, and drivers with medical restrictions provided in the Traffic Code.
Other recent changes related to the CNH
At the end of last year, the National Traffic Council (Contran) approved a resolution with other changes, such as ending the requirement for driving school classes to obtain the document, while maintaining the theoretical and practical exams as mandatory for issuing the CNH.
