Two explosions were heard just before the fire broke out on the Jag Leela, sending huge clouds of black smoke into the air.
Firefighters aboard another vessel battled to extinguish the blaze on the Indonesian-flagged ship, which was docked for repairs at the Belawan port in northern Sumatra.
Emergency personnel took 22 injured sailors to the hospital, with dozens more still trapped on the tanker, said local police chief Dayan, who shares the same name.
The cause of the explosion and fire has not yet been determined, he said.
The fire spread to at least one other vessel docked alongside the tanker, while the force of the explosion caused minor damage to nearby houses, Dayan added.
